An explanation of your service requirement (for example: this level has been dropped, please fix and re-calibrate or the laser needs calibration etc.).
Proof of warranty (invoice from purchase) if it is covered under warranty, and is to be repaired by the manufacturer. Shipping charges are not covered under warranty. We cannot guarantee turn-around time from manufacturer's repair department.
Tell us if you want an estimate first or just go ahead with the repair regardless of cost.

An estimate for repair is free, but if you decide not to get it repaired, you will be expected to pay return shipping. Unless you have an account with us, we will request payment the day we will return ship. We accept Visa, MasterCard, American Express, and Discover.
